Need a really really simple PHP app, that user will be able to upload a PDF file, and it will convert that file to PNG (keeping the same size aspect ratio) for each page separately.
Then, user has a split view interface, where in the left he has pages and right side a FORM with inputs that user can fill and save it into a json file into given directory, along with the current page using same name (for example, page_00001_89.json -for json, and same name for png as follows page_00001_89.png).

Unfortunatelly for this micro job, the budget is fixed but we have always a lot of jobs as this one for long term collaboration.
